If a participant, coach, or instructor has no fever and respiratory symptoms, they can attend the activity AFTER washing their hands and they will need to follow social distancing, frequent hand hygiene, and follow all of the CUSD guidelines.

If you answered YES to any of the questions above and/or have any of the symptoms above:

The Participant/Coach/Instructor needs to stay home and self-isolate until he/she is asymptomatic for three (3) days without the use of any medications, and it has been at least 7 days since the first day of their symptoms.

Please follow all health guidelines and preventative measures to stop the spread of germs. If you develop symptoms while at workouts, immediately self-isolate and notify your coach. When social distancing with a minimum of 6ft. is not achievable, athletes must wear face coverings. Click the 'submit' button below to send this questionnaire.
